A green punch mark and a current date on a fire extinguisher tag tell a building owner one thing: a technician serviced that unit within the past year. They do not say whether the cylinder has been opened for an internal exam in the past six years, or hydrostatically tested within its required interval, five years for some extinguisher types, twelve for others. NFPA 10, the National Fire Protection Association’s 2022 standard for portable fire extinguishers, sets four separate intervals, and most Kentucky facilities are current on exactly one of them without knowing it.
Four intervals, one tag
NFPA 10 (2022 edition) requires four nested checks on every portable extinguisher: a monthly visual inspection, an annual maintenance check performed by a qualified or certified technician, an internal examination every six years for stored-pressure units, and a hydrostatic test at intervals set by extinguisher type, five years for wet-chemical and CO2 units, twelve years for most dry-chemical stored-pressure units.
Two of these show up on the tag most people glance at: the monthly check, usually marked with initials and a date, and the annual maintenance, marked with a technician’s certification. The other two, the six-year exam and the hydrostatic test, run on a calendar most facilities don’t track separately, because nobody wrote the extinguisher’s manufacture date down anywhere else. An extinguisher can carry a fresh annual tag and still be years overdue for the interval that tests the cylinder itself.
What the annual tag covers, and what it skips
NFPA 10 requires the annual maintenance visit to be performed by a qualified or certified technician, who signs and dates the tag when the visit is complete. That signature certifies the extinguisher for another year of service.
It does not certify that the cylinder has been opened and examined internally, and it does not certify that the cylinder has been pressure-tested. Those are separate questions, answered only by the six-year internal exam and the hydrostatic test, and they don’t get answered just because the annual tag is current.
The six-year exam and the hydrostatic test that get forgotten
The six-year internal examination applies to stored-pressure extinguishers and involves emptying the unit to examine the inside of the cylinder, not just the outside. The hydrostatic test pressure-tests the cylinder itself, and how often it comes due depends on extinguisher type: five years for wet-chemical and CO2 units, twelve years for most dry-chemical stored-pressure units, under NFPA 10.
Because these intervals are counted in years rather than months, they lapse when a facility changes maintenance vendors, loses the paperwork on an extinguisher’s manufacture date, or assumes the current annual tag means everything is covered. An annual tag looks the same in year one as it does in year eleven. It doesn’t say anything about what’s overdue underneath it.
What happens when an extinguisher gets used
NAFED, the National Association of Fire Equipment Distributors, compiled four surveys of real-world extinguisher use conducted between 1976 and 2010: 32,756 extinguishers deployed across 13,453 incidents, 13,221 of them confirmed fires. Extinguishers put the fire out in 95 percent of those fires, 12,505 of 13,221. In the 2010 survey alone, 10,432 extinguishers were used in 4,633 incidents (4,401 confirmed fires), also at a 95 percent extinguishment rate, and a single extinguisher was enough to finish the job in 72 percent of those fires.
This is the newest data of its kind. NAFED has not published a survey of comparable scale since 2010, and no more recent one of this size has been found. The 2010 compilation, now sixteen years old, remains the best available answer to how extinguishers perform in a real fire, not a training drill.
Across all four surveys, the leading reason an extinguisher failed to fully put out a fire wasn’t a bad valve or a dead cylinder. It was that the fire had already grown too large by the time someone reached for the extinguisher. The equipment performed in most of those cases; the delay in using it didn’t. That points toward training people to reach for an extinguisher early and correctly, not just keeping one mounted on the wall.
The penalty math doesn’t care whether extinguishers make headlines
Federal OSHA’s maximum penalties, effective after January 15, 2026, are $16,550 per violation for serious, other-than-serious, or posting violations; $16,550 per day beyond the abatement deadline for failure to abate; and $165,514 per violation for willful or repeated violations.
OSHA’s own FY2025 Top 10 list of most frequently cited standards, covering federal inspections from October 2024 through September 2025, runs fall protection, hazard communication, ladders, lockout-tagout, respiratory protection, scaffolding, fall protection training, powered industrial trucks, eye and face protection, and machine guarding. Portable fire extinguishers (1910.157) and exit routes (1910.33 through .39) are not on it.
That absence is easy to misread as low risk, but a standard doesn’t need to crack a national top 10 list to get cited during an inspection, and the penalty schedule above applies the same whether the citation is for a lockout tag or an expired extinguisher tag. Kentucky enforces workplace safety through its own state plan rather than federal OSHA directly. State plans are required to adopt penalty levels at least as effective as federal OSHA’s, but they are not required to match the federal amounts exactly, so read the figures above as the federal schedule rather than Kentucky’s own. In federal fiscal year 2024, the Kentucky Division of OSH Compliance issued $4,245,600 in penalties across 1,097 citations from 740 inspections, according to its SOAR report to federal OSHA. An inspector already on-site for something else doesn’t skip the extinguisher closet because it isn’t the headline violation.
A monthly walkthrough any staff member can run
NFPA 10’s monthly inspection doesn’t require a technician or a certification. It requires someone who knows what to check and a minute per extinguisher. A walkthrough should confirm:
- The extinguisher is in its assigned location and visible, not blocked by boxes, equipment, or furniture.
- The pressure gauge needle sits in the operable range, not below it.
- The safety seal and pin are intact and unbroken.
- The extinguisher shows no obvious damage: dents, corrosion, or a cracked or clogged hose.
- The inspection gets logged, with initials and a date, whether that’s on a paper tag, a log sheet, or a barcode scan.
None of this replaces the annual maintenance visit or the six-year and hydrostatic intervals. It catches the extinguisher that’s been moved, blocked, or damaged since the last technician visit: the kind of problem most likely to show up between scheduled service dates.
What this looks like handled well
An extinguisher with a current annual tag and an overdue six-year exam looks compliant on paper, even though the underlying interval has already lapsed. The gap is common because the two intervals don’t share a calendar and nobody is tracking them side by side.
